Name Change Procedures

Licensees who have had a legal name change must submit one of the following official documents to update your license/practitioner information:
  • Certified marriage certificate;
  • Final court documents supporting the name change;
  • New social security card; or
  • New drivers license.

If you would like a new certificate prior to renewing your annual license, a $10 fee will be assessed.

This payment can be processed via:
1. Online payment portal. A convenience fee of $.58 will be applied.
2. US Postal Service.
